General Instructions:
1. Include registration fee of $3 per puppy members, $5 per puppy non-members,
payable to AWF secretary, Erin Hischke. This can be check or money order.
2. Information that is helpful but is not required:
a. List of names and addresses of puppies new owners.
b. Any health testing that was done on or in preparation for this litter.
c. Copies of the sire and dam's pedigree. (can be sent by email).
Section A: Owner/Breeder Identification-
The breeder is the person who owned the dam at time of service. The Owner may be
a different person if the ownership of the dam was transferred after service.
The applicant must be the current owner of the dam.
Section B: Litter Identification-
1.At least one parent must have an AWF number. If one of the parents is not
registered with the AWFA, write N/A in the appropriate space. Include other
registration numbers if available. Any unregistered parent must be of a collie
breed or mix thereof, or appear to be if ancestry is unknown. Two photographs of
unregistered parent must be included: a side shot and a full face shot.
2. Please assign a puppy name and number to each puppy in the litter. This may
not necessarily be the puppy's permanent name, but will be needed by the new
owner for identification purposes when a permanent registration is applied for.
